Pharmacy and Prescriptions

When your provider prescribes medication for you, it's very important that you take the medicine exactly as prescribed. We are all blessed to have such a broad range of prescription drugs available these days that are incredibly effective in treating illness and managing chronic disease. But getting you better is always a partnership that requires your provider to prescribe the right medication and you to faithfully take it.

Your pharmacist can help you by making sure you understand your prescription, how to take it, when to take it and what to expect in terms of any side effects. At GraceMed, we have included an in-clinic pharmacy at three of our largest clinics to make it easier for you to get your prescription filled. We have also found that it improves the quality of your care to have a pharmacist on the same team with your provider to make sure you achieve the best possible outcome from your treatment regimen.

Help with the cost of your prescriptions

Of course, the cost of today's pharmaceuticals can often make it hard to afford them, especially if you don't have insurance coverage. That's why GraceMed offers the Prescription Assistance Program. It helps qualified patients take advantage of plans offered by a number of pharmaceutical companies to get their medications at no cost to them.

The program requires you to go through an application process with each new prescription you receive. GraceMed will help you complete your application. We will also provide samples, as available, to get you started while your application is being processed. The application process typically takes about one month to complete. If samples are not available, you may have to purchase a limited supply at an area pharmacy.

There are some requirements you have to meet in order to qualify for the Prescription Assistance Program.

You are required to:

  • Keep all appointments

  • Provide written documentation of income as stipulated by the pharmaceutical companies

  • Sign your application form within 10 days of receiving the notification

  • Update any change of address or phone number

  • Provide truthful information

Samples are provided by prescription only, and only your provider can write your script.
