Integrated Care:  What is it?

A team of primary care and behavioral health clinicians working together with patients and families to provide patient-centered care. This care may address mental health and substance misuse conditions, health behaviors (including their contribution to chronic medical illnesses), life stressors and crises, and stress-related physical symptoms. 

Behavioral Health Consultants (BHCs) are responsible for providing behavioral health consultation to children, adolescents, adults, and families in order to improve psycho-social functioning.

BHCs work with your primary health care providers in recognizing and treating mental disorders and psychosocial problems.

Telehealth

Treating behavioral health conditions is a process, one that often develops over long stretches of time. We know that making that commitment can be difficult in the middle of a busy, stress-filled life. That’s why we’ve established the capability to consult with you through our telehealth communications system that lets our consultants see you at the GraceMed clinic that is most convenient for you. We’ve secured the technology and dedicated a room in each of our clinics to these remote sessions to ensure your complete privacy.
