GraceMed Health Clinic's Homeless Healthcare Outreach Program is a vital initiative designed to ensure that homeless individuals have access to quality healthcare. Led by our dedicated Homeless Outreach Nurse, Elana Hauck, who brings over 30 years of experience working with homeless populations, the program delivers essential medical services directly to those in need. We have established strong partnerships with organizations such as Union Rescue Mission, United Methodist Open Door, Salvation Army, Catholic Charities, HumanKind, and Topeka Rescue Mission, as well as numerous agencies across the state.
Healthcare is critically important for the homeless population. Without access to regular medical care, homeless individuals are more likely to suffer from untreated chronic illnesses, acute medical conditions, and mental health issues, which can exacerbate their circumstances and hinder their ability to find stable housing. By providing access to healthcare, our program helps to address these issues early on, preventing health problems from becoming severe and reducing the overall burden on emergency services.
Having a primary care provider is essential for homeless individuals as it ensures continuity of care, regular health monitoring, and a trusted point of contact for medical concerns. Our outreach program provides comprehensive healthcare services, including medical care, dental care, podiatry, and behavioral health support. We also connect homeless patients to pharmacy prescription programs, vision care, and assistance with applying for Medicaid and Medicare.
Access to healthcare can play a pivotal role in preventing homelessness by stabilizing individuals' health, making it easier for them to seek employment and secure housing. By addressing both immediate health needs and long-term care, we aim to improve the overall well-being of homeless individuals and support them in their journey towards stability and self-sufficiency.
