Community Care Services
Recovering lost lives is good for all of us.
There are times in life when too many of us can find ourselves on the outside looking in. We're completely alone for reasons that are too hard, too painful to explain. When that happens, bad luck can turn to worse luck in what may seem like no time at all, and suddenly you're more vulnerable and afraid than you ever imagined you could be.
GraceMed provides our Community Care services as a resource for people out on the margins of life, those who are without a home or a means to get one and those who have problems, legal and otherwise, that can become insurmountable roadblocks to recovering their health and lives.

Medical Outreach to the Homeless
In Wichita, GraceMed also collaborates with local homeless shelters to provide nursing care for the homeless. Our licensed Register Nurse (RN) visits residents of Open Door and other homeless shelters and drop-in centers to assess medical and dental needs. Referrals are made to clinic-based services for conditions like diabetes, asthma, depression and substance abuse.
