Refer to the “contact us” page to find clinic contact information. Call any of the phone numbers listed to speak to our friendly staff about making an appointment.

 
How am I charged for my visit? PDF Print E-mail

Services are charged on a sliding fee scale based on household income. Medicare, Medicaid, HealthWave and commercial insurance plans are also accepted

We do not accept personal checks. Please pay with cash or money order only. Credit and Debit cards are accepted at the Topeka location only.

 
What should I bring to my visit? PDF Print E-mail
  • Photo identification
  • Proof of all income in your household to receive a discount. Proof of income includes the following:
    • Paycheck stub
    • Last tax return
    • Notarized statement from your employer
    • Statement of benefits from the Social Security program or other government assistance program
 
What if my healthcare need is not listed? PDF Print E-mail

Please call one of our clinics about your specific healthcare need. If we are not able to provide a service we may be able to refer you to a provider who can.
